Today I received an email notification that I had a PM awaiting me, but when I went to check, there was no sign of a message. Has this happened to anyone else?

Scott, your email address bounces back to me. The issue is solved, the sender didn't know how to use the system and sent it via email.

Is there a way to attach a file to a private message reply?

Nope. The PMs go via the board software. Should you wish to trade files with someone via attachments use your PMs to get real e-mail addresses from each other.
